OFF THE MARK RELEASE + INDEMNITY

OFF THE MARK shall not be held liable for personal injuries sustained by the above named athlete.  The athlete assumes full responsibility for any injuries or damages which occur to the athlete in or around the vicinity of the training venue occupied by OFF THE MARK. The athlete hereby fully and forever releases and discharges OFF THE MARK, its owners, employees and agents from any and all claims, demands, rights or action or causes of action, present or future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the athlete’s use of the facilities occupied by OFF THE MARK or the equipment thereof, including, but not limited to any claims or personal injuries resulting from or arising out of the negligence of the OFF THE MARK employees, the associated Rustenburg Girls' High School, City of Cape Town and Alphen Greenbelt, or its owners, employees or agents, or the negligence of any other persons, present on the premises including other persons sincerely using the premises.
 
I have read this waiver and understand the risks associated with my participation or my child’s participation in any activity associated with the OFF THE MARK training program. By my acceptance hereof I fully understand and accept that upon registration as an athlete all activities and obligations undertaken as such are entirely at my own risk and that I am physically able and fit to participate in the program.

I grant OFF THE MARK full permission free of charge to use my name as well as any photographs and footage taken of me during my participation in the program for use in advertising, promotions or other account of it.

I further understand that it is required of me to sign this waiver in order for me or my child to have full participation in the OFF THE MARK training program and that I do so voluntarily.
